HAZARDS ASSESSMENT SERVICES: EARTHQUAKE AND VOLCANIC HAZARDS
DOST-PHIVOLCS Hazard Assessment Services (HAS) is one of PHIVOLCS’ frontline services
that determines if any earthquake and/or volcanic hazard is present in a particular
parcel of land of interest to a stakeholder,
the result of which is contained in a
Hazard Assessment Report. It is also responsible for providing hazard information
packages requested by stakeholders.
Hazard Assessment Report (HAR) – the document that contains the hazard
assessment for a particular parcel of land of interest to a stakeholder.
The HAR by
itself is NOT an approval, endorsement or authorization for further government
procedure.
This system is supported by GeoRiskPH.
All hazard information are from DOST-PHIVOLCS. Hazard information may be refined as new data become available.
DOST-PHIVOLCS collects personal data through this form for purposes including service delivery, stakeholder engagement, policy formulation, program implementation and other processes as required by law including auditing.
By filling out the Hazard Assessment Request form, you agree to the stated uses of your data. Rest assured that DOST-PHIVOLCS is protecting your personal data in adherence to the Data Privacy Act of 2012.
NOTICE OF SYSTEM MAINTENANCE:
Please note that our Online Hazard Assessment Service System is experiencing latencies due to system maintenance.
You may still file your request/s online but there may be some delays in the processing.
We apologize for this inconvenience. This notice will be lifted once system maintenance is done.
TERMS AND AGREEMENT:
***This site uses sessions, cookies and third-party service to analyze non-identifiable web traffic data. PHIVOLCS is committed to protect and respect your personal data privacy in compliance with the Data Privacy Act of 2012.
***DOST-PHIVOLCS categorizes requests for Hazard Assessment Reports based on three categories. Simple (3 days), Complex (7 days) and Highly Technical (20 days). Stakeholders will be notified accordingly.
***DOST-PHIVOLCS will communicate with you once documents are found insufficient to begin hazard assessment.
***Requests for Hazard Assessment placed on weekends and holidays will be processed on the next business day.
***Fee: Php 500 for brokers and developers.
***Fee: Php 100 for homeowners.
***Free to government agencies and LGUs.
Request online for signed Hazard Assessment Reports (HARs) through DOST-PHIVOLCS' Online Hazard Assessment Service (OHAS) platform: has.phivolcs.dost.gov.ph.
HARs requested by private entities/individuals will be released once payment is made through the DOST-PHIVOLCS cashier or online via the LANDBANK Link.BizPortal. To pay online:
1. Go to https://www.lbp-eservices.com/egps/portal/index.jsp then click Pay Now.
2. Select PHILIPPINE INSTITUTE OF VOLCANOLOGY AND SEISMOLOGY as Merchant for your payment and click Continue.
3. Select Hazard Assessment Certification as the transaction you wish to pay. Click Continue.
4. Input necessary information in the Payment Details and select Payment Mode then click Continue.
Please indicate in the "Name:" the Company Name or Lot Owner's Name.
(Please note that, currently, we only accept payment modes via Landbank/OFBank ATM Card and Cash Payments via 7/11 and Cebuana. Other payment channels are still unavailable)
5. Please upload a copy (pdf or screenshot) of the Receipt as proof of payment in the OHAS portal (https://has.phivolcs.dost.gov.ph) using the Tracking No. provided in the reply email or send us a proof of payment via email at ggrdd.mail@phivolcs.dost.gov.ph.
6. Once payment has been verified by PHIVOLCS Staff, you should be able to download the Hazard Assessment Report in the OHAS portal.
For requests made by government offices, HARs may be downloaded as soon as they are available.
